What is DIM?
3,3′-Diindolylmethane (DIM) is a condensation product of indole-3-carbinol formed during digestion of cruciferous vegetables. Capsules, however, provide doses and formulations very different from a meal.
Human evidence mostly shows changes in hormone metabolites and pharmacokinetics. It does not establish that DIM prevents or treats cancer, acne or a hormone imbalance. In the most relevant interaction trial, DIM lowered active tamoxifen metabolites. [1]
Typical DIM Protocols
| Source and context | Form | Amount | Frequency | Duration | Observed result |
|---|---|---|---|---|---|
| Thomson 2017 · women taking tamoxifen | Absorption-enhanced BR-DIM | 150 mg | Twice daily | 12 months | Urinary 2/16α-OHE1 ratio and SHBG increased; breast density unchanged; endoxifen and other tamoxifen metabolites decreased 130 randomized and 98 completed; primary endpoint was a surrogate, not cancer recurrence. |
| Castañon 2012 · low-grade cervical cytology | BioResponse DIM | 150 mg | Once daily | 6 months | CIN2+ in 8.8% versus 12.4%; confidence interval crossed 1; neither cytology nor HPV clearance improved 551 women analyzed in a double-blind 2:1 randomized trial; broadly null clinical result. |
| Heath 2010 · castration-resistant prostate cancer | Microdispersed BR-DIM | 75, 150, 225 or 300 mg | Twice daily in 28-day cycles | Median 4.0 months; range 1.0 to 18.5 | One 50% PSA decline, one stabilization, 10 later progressions; hyponatremia at highest dose Noncomparative phase 1 in 12 men; designed for dose, toxicity and pharmacokinetics, not proof of efficacy. |
No RDA, AI or UL exists. Experimental oncology doses are not nutrient requirements.

What is DIM commonly used for?
DIM is sold for “estrogen balance,” acne, menopause, prostate health and cancer prevention. Trials do not validate those broad promises. Thomson showed a urinary-metabolite change, not a better cancer outcome; Castañon did not show useful improvement in cytology or HPV. [1][2]
The antiandrogen signal in the small prostate study is exploratory. One PSA decline in an uncontrolled phase 1 cannot establish treatment benefit. [3]
Which forms and labels matter?
BR-DIM is an absorption-enhanced microdispersed formulation used in trials; crystalline DIM capsules are not milligram-for-milligram interchangeable. Indole-3-carbinol (I3C) is a precursor that forms several products in acid, not DIM alone.
A label should state actual DIM per dose, formulation, excipients and other hormone-active ingredients. Black pepper or “detox” blends further change exposure and interaction risk.
What does the human evidence show?
In Thomson, 130 women taking tamoxifen were randomized and 98 completed 12 months. BR-DIM increased the 2/16α-OHE1 ratio and SHBG without changing breast density. It also lowered endoxifen, 4-OH-tamoxifen and N-desmethyl-tamoxifen, creating a clinically important interaction signal whose effect on tamoxifen benefit remains unknown. [1]
Castañon analyzed 551 women with low-grade cervical cytology. CIN2+ occurred in 8.8% with DIM and 12.4% with placebo, RR 0.70, 95% CI 0.4 to 1.2. HPV persisted in 68.7% versus 60.6% of those positive at baseline. The authors concluded 150 mg daily was unlikely to improve cytology or HPV infection. [2]
Heath treated 12 men across cohorts of 75 to 300 mg twice daily. Exposure increased with dose; two of four men at 300 mg developed grade 3 asymptomatic hyponatremia. Ten of 12 eventually progressed. [3]
How might DIM work?
DIM can activate the aryl hydrocarbon receptor and change expression of enzymes that process estrogens. A changing urine ratio shows different metabolism; it does not prove lower cancer risk.
In human hepatocytes and intestinal cells, DIM activated PXR and increased CYP3A4 and MDR1 expression. This mechanism is consistent with the clinical signal involving tamoxifen metabolites, although it cannot predict every interaction. [1][4]
What are the risks and interactions?
Headache, nausea and digestive discomfort are reported. Isolated reports describe visual impairment, drug reaction with eosinophilia, stroke, pulmonary embolism and deep-vein thrombosis; they identify possible signals without proving causation. [5]
Avoid during pregnancy and breastfeeding. Anyone using hormonal contraception, hormone therapy, tamoxifen or a CYP3A4/MDR1 substrate should have the interaction reviewed before use. Cancer or another hormone-sensitive condition requires the treating team’s input.
Regulatory status in the United States and Canada
DIM is sold as a dietary supplement in the United States and may appear in natural health products in Canada. Commercial status does not authorize cancer, HPV or hormone-disorder treatment claims.
What should be monitored?
Record product, formulation, amount per dose, frequency, hormone medicine, symptoms, sodium when medically monitored, laboratory dates and stopping reason. A urine ratio or PSA should not be interpreted alone as proof of benefit.
What happens after stopping?
No withdrawal syndrome or taper is established. Review an enzyme interaction or adverse effect with the clinician; do not automatically restart.
How should it be stored?
Store closed, dry and away from children. Keep lot and formulation with the record.
Community experiences
Acne and “estrogen dominance” stories often use nonstandard diagnoses and change several variables together. A temporal improvement shows neither mechanism nor safety with a hormone-active medicine.
How does DIM compare with other approaches?
Cruciferous vegetables provide fibre, nutrients and many compounds; they are not equivalent to a BR-DIM capsule. I3C forms a variable mixture of gastric condensation products. Prescribed endocrine treatments have defined indications, doses and monitoring that DIM does not replace.
Frequently asked questions
Does DIM balance hormones?
It changes selected metabolites and SHBG, but “hormone balance” is not a defined clinical outcome. [1]
Can it be taken with tamoxifen?
The trial found lower endoxifen and other metabolites; discuss use with the oncologist or pharmacist first. [1]
Does it treat HPV or cervical abnormalities?
150 mg daily for 6 months did not improve cytology or HPV clearance in the large trial. [2]
Are the prostate doses wellness doses?
No. They came from a small phase 1 in men with castration-resistant cancer. [3]
Are food-derived DIM and supplements equivalent?
No. Food formation varies, and absorption-enhanced formulations change exposure.
Should sodium be monitored?
Grade 3 asymptomatic hyponatremia occurred in two of four men at 300 mg twice daily; monitoring depends on medical context. [3]
